1. When to consider surgery as a treatment option for vitiligo ?
- Surgical treatment can be considered when:
- The patient has undergone adequate medical management (e.g., phototherapy, topicals) for at least 12 months with little or no improvement.
- Minimal or no repigmentation despite consistent treatment.
- Surgery is often more successful and preferred in segmental vitiligo, which stabilizes earlier and responds less predictably to medical treatment.
- Surgery is also preferred in cosmetically sensitive areas (e.g., lips, face, eyelids), when the patient seeks a faster or more definitive solution.
2. What are the prerequisite before opting for vitiligo surgery ?
Disease Stability is the single most important criterion before and patient is eligible for surgery. This means :
– No new lesions in ≥ 6 months
– No increase in size of old lesions
– No new lesions at sites of trauma/ injury
3. What are the different surgery options available for vitiligo ?
Here are the main categories of surgical treatments for vitiligo:
1. Tissue (Mini-Grafting) Techniques
a) Punch Grafting
Small punches of pigmented skin (1–2 mm) are taken and transplanted into depigmented areas.
Simple and cost-effective.
However only small areas of vitiligo can be treated with this technique and often multiple sessions are required
Risk: cobblestoning, halo effect, polka-dot appearance.
b) Suction Blister Grafting
Suction is applied to donor skin to create blisters.
Epidermis is harvested and placed on dermabraded recipient area.
Natural pigmentation with fewer textural issues.
Time-consuming but effective for small/moderate areas.
This is a very effective technique to treat lip vitiligo.
This technique cannot be used over large areas and may require multiple sessions to treat large areas.
c) Split-Thickness Skin Grafting (STSG)
Thin layer of pigmented skin is taken ( usually from the thigh) and grafted onto the affected area.
Not suitable for large areas.
2. Cellular Transplant Techniques
a) Melanocyte–Keratinocyte Transplantation Procedure (MKTP)
Advanced and refined technique combining melanocytes and keratinocytes in suspension.
A small skin sample is enzymatically digested to isolate melanocytes and keratinocytes.
Cell suspension is applied to a dermabraded vitiligo patch.
Allows treatment of larger areas from a small donor site.
Rapid repigmentation, uniform tone.

1. Large Area Coverage with Minimal Donor Skin
A small donor area (e.g., 2–10 cm²) can treat up to 10 times or more of the area.
This is especially beneficial in patients with extensive vitiligo but limited donor skin.
2. Excellent Cosmetic Outcomes
Uniform pigmentation and smooth texture.
Minimal risk of cobblestoning, polka-dot appearance, or textural mismatches seen with punch grafting.
Superior aesthetic outcomes on face, neck, and visible areas.
3. Minimal Scarring
Both donor and recipient sites usually heal well with minimal to no scarring, especially when proper technique is followed.
Ideal for patients with cosmetic concerns.
4. Rapid Repigmentation
Initial repigmentation often begins within 2–4 weeks, with excellent results by 3–4 months in successful cases.
5. Stable and Long-lasting Results
In stable vitiligo, the pigmentation achieved is typically durable.
Recurrence is uncommon if stability criteria are properly met pre-surgery.
6. Versatility
Effective on difficult-to-treat areas like hands, feet, lips, eyelids, and joints.
Can be used across various skin types and vitiligo subtypes (segmental, focal, vulgaris).
7. Lower Post-Op Downtime
Healing is faster than with split-thickness grafts or punch grafts.
Less postoperative pain and minimal immobilization required.
